Ramp Application

Thank you for your request to be considered for the Ability 1st Accessibility Home Modification Program.

We build ramps and half-steps for persons unable to access their home safely. In some circumstances we are able to
provide other home modifications pending available resources.

Please complete and sign the enclosed application to be considered for our program. Proof of income for each member of the household is required for your application to be processed. 

Please note that if you do not own your home, a letter from the home owner or landlord granting permission for Ability 1st to modify the home is required before you can be considered
for the program.

Examples of proof of income: Social Security Award Letter, bank statement, or check stubs, child support documentation, etc.

*If you live alone and are receiving Medicaid benefits, a copy of your Medicaid card will serve as proof of income*

Your completed application will be processed in the order it was received.

After your application has been reviewed, an Ability 1st staff member will contact you to let you know if you are approved and what, if any, share of cost will be required from you.

Please be advised that the Ability 1st Accessibility Program covers Leon, Gadsden, Wakulla, Taylor, Jefferson and Madison Counties.
We have wait lists in each of the 6 counties we serve.
