What is it?
In our outreach efforts, we identify disabled individuals diagnosed with, or exhibiting symptoms of severe and persistent mental illness, substance abuse, or dual disorder, and help them navigate the maze of community treatment and resources.
How does it help?
We help guide and link our consumers to housing options, treatment services, and other resources needed to achieve their independent living goals. Eligible individuals may receive guidance on accessing critical benefits such as SSI, SSDI, Medicaid, and SNAP (formerly known as the food stamp program).
Ability1st is also an Access Point provider for people experiencing housing crisis. As a provider, we can conduct a coordinated assessment to screen for permanent supportive housing eligibility and provide information and referrals, as appropriate.
Upon assessment, we may also provide or refer for immediate needs such as food, personal hygiene items, and other critical supplies.
Who is it for?
Adults 18 years of age or older with a defined mental health, substance abuse or co-occurring disability.
