Core Service: Nursing Transition

Ability1st is dedicated to supporting individuals with disabilities so that they may thrive in a community-based living environment. Our Nursing Home Transition services are embedded in our Access to Independence Program. We focus on assisting individuals with disabilities who wish to avoid placement in nursing home or other more restrictive institutional settings or those looking to transition back into the community. We believe in the right of every individual to live in the least restrictive environment possible.

Services include:

  • Planning and coordination of services to support living in the community, including construction of ramps and half steps, durable medical equipment loans, and incontinence products.
  • Advocacy and assistance in navigating systems, funding sources, and legal rights.
  • Support and guidance for families and caregivers to ensure a smooth transition process.

Building a Future of Independence and Choice

he overall goals are to improve home accessibility of vulnerable adults with disabilities who are at risk of institutionalization and improve the overall mobility and self-care capacity of adults with physical disabilities. We not only positively impact our Consumers but their families as well.

Who Benefits?

Adults with disabilities residing in Leon, Wakulla, Madison, Taylor, or Gadsden counties may apply. Program funding is limited to serving individuals with low to moderate income.

The goal of all Centers for Independent Living is to empower people with disabilities to take charge of their lives and guide their own destinies. We are a community-based non profit organization that provides services to people with varying disabilities in Leon, Jefferson, Madison, Taylor, Gadsden, and Wakulla counties.
