Core Service: Nursing Transition
Ability1st is dedicated to supporting individuals with disabilities so that they may thrive in a community-based living environment. Our Nursing Home Transition services are embedded in our Access to Independence Program. We focus on assisting individuals with disabilities who wish to avoid placement in nursing home or other more restrictive institutional settings or those looking to transition back into the community. We believe in the right of every individual to live in the least restrictive environment possible.
Services include:
- Planning and coordination of services to support living in the community, including construction of ramps and half steps, durable medical equipment loans, and incontinence products.
- Advocacy and assistance in navigating systems, funding sources, and legal rights.
- Support and guidance for families and caregivers to ensure a smooth transition process.
